How can I access my own medical records?
If you would like to access your own health information or records, you have a right to request this by contacting the health service provider with whom your information is being held. This may be your GP, specialist or a hospital where you are or were a patient.
Can I read my medical notes in hospital?
You won’t be allowed to read anything in your records that’s about another person (unless they give permission) or that might cause you serious harm. You also have the right to ask that other people (including your parents or carers) are not allowed to see your health records, if you want.
